Summary:
Provide care and support to patient on a day/night full time basis.
The position may be result in a living arrangement in the home of patient on a partial or full time basis.
Care and support include both physical and emotional needs.
PRIMARY DUTIES AND RESPONSIBILITES:
1.
Adhere to structured daily schedule for patient.
2.
Manage the proper movement of the patient in and out of beds, baths, wheelchair, etc.
3.
Assist the patient in grooming, dressing and other personal care.
4.
Light housekeeping, to include washing and ironing laundry, changing bedding and cleaning.
5.
Maintenance and administration of medicines.
6.
Flushing 7.
Plan, purchase, prepare and serve meals according to dietary needs.
8.
Protect privacy and confidential information.
9.
Entertain, converse, read aloud, play games, etc.
10.
Provide transportation, assistance and companionship by accompanying patient to appointments (shopping, physician appointments, hair appointments, etc.
) 11.
Ensure patient engages in daily exercise as deemed necessary.
12.
Organize mail that may arrive at the home location.
13.
Keep daily journal of activity, logging in computer.
PERSONAL AND ORGANIZATIONAL SKILLS:
1.
Personable, patient, cheerful, and engaging.
2.
Strong organization skills and very structured in compliance with schedule and tasks.
3.
Strong willed to ensure expectations are met.
4.
Communication skills - both verbal and written (must be able to articulate with patient and family in every situation.

Responsibilities:
1.
Ability to travel approximately 25% of time.
2.
Working knowledge of computer to utilize programs for logging activity, hours worked, etc.
3.
Physical demands:
Lift/Carry (in proper manner with equipment provided) Standing, bending, walking, and sitting Strong grasp Climb, squat, and kneel EDUCATION AND
Experience:
1.
Bachelor's degree preferred, but not required; (minimum of high school degree.
) 2.
CNA Certification required 3.
Experience working with MS patients preferred, ability to recognize symptoms (including but not limited to; UTI) 4.
Experience working with patients with disabilities or incapacitating illnesses Job Type:
